Instructions below!
  1. Create a drive folder for each of your students. Copy each of the data sheets (weekly and quarterly) into the folders, and input their IEP goals and objectives into the sheet. Adjust the key to reflect the goal.
    1. EXAMPLE- 1: Given visuals and manipulatives, Student will match numbers to pictures of their quantities with 80% accuracy in 4 out of 5 trials as measured by data collection.
      1. The key for this would be + or -. I typically provide 5 or 10 opportunities per trial. This means that each opportunity given 5, is worth 20% or given 10, each opportunity is worth 10%. Because I write all my goals with x out of 5 opportunities I know that each + is 20%. So if that student completes the activity to 80% 3 times that week the weekly sheet would read as follows:

  1. EXAMPLE 2: Using letter sounds knowledge, Student will be able to read CVC words, with 90% accuracy, in 3 out of 4 trials, as measured by data collection, by the end of 36 academic weeks.
    1. Sometimes a goal is not written in a way that + or - accurately documents the student’s knowledge. This is when I would alter the key. For the goal in example 2 I would use an x/10 in each trial. This means that at each trial I offer 10 opportunities and document the number correct out of 10.

To calculate this weekly percentage I would take the total number of correct opportunities and divide that number by the total number of opportunities. In this case, it would look like 4+6+5+7+9= 31 31/50= 0.62= 62%




  1. Once the goals, objectives, and keys are completed print the weekly sheet. I usually print enough for the whole quarter.



  1. At the end of the week copy the percentages over into the corresponding space on the quarterly sheet. To calculate the goal using the objectives, average the weekly percentages for each objective.

  1. At the end of the quarter you will have all data in one place to use for progress reports or IEP renewals!
